Must own and occupy subject property as of July 1, 2012

Must have continuously lived in Massachusetts for the preceding TEN years

Must have owned and occupied property in Massachusetts for the preceding FIVE years

THE FOLLOWING INFORMATION MUST BE PROVIDED IN ORDER TO QUALIFY

COPIES OF:

1. Bank and all other asset account statements as of July 1, 2012
including, but not limited to, value of checking, savings, stocks, bonds, CD’s, etc.

2. Social Security Statement from preceding year (2011)

3. Pension Statements from preceding year (2011)

4. Income Tax Returns from previous year (2011) *

5. First time applicants must provide a copy of Birth Certificate
